Minor Client Consent & Liability Waiver

Important Notice: Parental/Guardian Consent Required

If the client is under the age of 18, a parent or legal guardian must give permission for any lash or brow services to be performed. No services will be done without this signed consent form.

By signing this form, the parent/guardian agrees to the following:

• I give permission for La Casa Mely to perform lash and/or brow services on my child.

• I understand the nature of the services and have been informed of possible risks (e.g., irritation, allergic reaction, lash fallout, etc.).

• I release La Casa Mely from any liability resulting from services performed with my consent.

• I understand that if my child receives services without my consent, La Casa Mely is not responsible, as long as the form was completed honestly by the minor or falsely presented by the client.

Service Agreement

• All health concerns, allergies, or sensitivities must be disclosed before the appointment.

• If your child is experiencing any discomfort, irritation, or changes in health, you must notify La Casa Mely within 48 hours of the service.

• After 48 hours, La Casa Mely is not liable for any reactions or concerns.
