PLEASE READ BEFORE FILLING OUT FORM!! This form has been created to ensure that all members of the RSO Cardinal Confections are made aware of allergen and general food safety rules before being allowed to bring homemade baked goods to an event. By filling out this form you are confirming that you have been informed on all major allergens and food safety rules. All members who are interested in bringing a baked good MUST fill out this form before the event, it is mandatory. All members will also be required to list all ingredients used. This for the safety for both the bakers and to consumers. Thank you! :)

This form is required for all students wanting to participate in activities and events hosted by Cardinal Confections.
