Application Information: Due Date: This Google Form Application and two Teacher Recommendation forms must be submitted NO LATER THAN Friday, April 14th at 2:30pm in room 219. No late applications will be accepted! Teacher Recommendation: You are responsible for asking two JFK teachers to recommend you for Peer Counseling. Please input the name of the teachers that plan to submit the recommendation form for you in the space provided at the end of the application. Pick up the recommendation paper forms from Mrs. Callon in room 219 or 217.Your teacher recommendation forms will be submitted separately from this application. Interview: We will be conducting interviews on campus. You will be contacted via email and/or Remind text to schedule a date and time for the interview.
