`publish` to Posit Connect breaks on large titles

the name of a document needs to be from 3 to 64 chars in Posit Connect, and we break this.

ERROR: API Error: 400 - Bad Request

Two related problems:

• We need to create a truncated name
• We need a better error message for failures. Connect uses the response payload for their error messages, and we ignore that.
